Winter 2023 Athletics

In order to participate in Winter Sports, please complete the following steps: 

1. Register on app.formreleaf.com

2. Complete the Following Paperwork: 

   1. 2023 Physical Examination Form 

• To view and print these forms, please click here.
• Physical Forms for the 2023 - 2024 school year must be dated after June 7, 2023.  

• ONLY the Preparticipation Physical Evaluation form should be returned to the Athletic Director.  

• The History and Physical Examination Form should remain with the Physician.

Paperwork can be dropped off at the Main Office before November 15, 2023.
