Spring 2024 Athletics

In order to participate in Spring Sports, please complete the following steps: 

  1. Register on app.formreleaf.com

  2. Complete the Following Paperwork:

    1. 2023 Physical Examination Form

  • To view and print these forms, please click here.
  • Physical Forms for the 2023 - 2024 school year must be dated after June 7, 2023.  
  • The Preparticipation Physical Evaluation form should be returned to the Athletic Director.  
  • The History and Physical Examination Form should remain with the Physician. 
  • Team managers only need to submit the Preparticipation
    Physical Evaluation Form.

 


Tryouts start March 1, 2024

Paperwork can be dropped off at the Main Office.
